Cyanocobalamin injection, the B12 shot, is 1000 mcg/mL. One common manufacturer sells it as a 1 mL vial, a 10 mL multiple-dose vial, and a 30 mL multiple-dose vial (American Regent label). It's given intramuscularly or subcutaneously. The label says to avoid the IV route.
The 30 mL vial has the lowest price per milliliter. Whether it's the cheapest one for you depends on one number: how much you use in the 28 days after you open it.
What sizes does it come in?
From American Regent's prescribing information (label):
Other manufacturers sell similar sizes with different NDCs. Check the label on the product you actually buy.
Why does the 30 mL vial cost more than it looks?
Because of the 28-day rule. Once a multiple-dose vial is opened, CDC says to date it and discard it within 28 days unless the manufacturer states another date (CDC). That means writing the discard date on it, not the date you opened it (The Joint Commission).
Do the math before you buy:
- If you use less than 30 mL in 28 days, whatever's left in a 30 mL vial on day 29 is waste. Compare the price per mL you use, not the price per mL you buy.
- If the vial goes into the exam room, CDC says it's that patient's vial from then on (CDC). A practice that draws up B12 at the bedside may waste less with smaller vials.
- If you give a lot of B12 from a clean prep area, the 30 mL vial can be the right call.
We walk through the full set of rules in single-dose vs. multiple-dose vials.
What's in the vial besides B12?
Two things on the label are worth knowing (American Regent label):
- Benzyl alcohol, 1.5%, as a preservative. The label warns that benzyl alcohol "has been reported to be associated with a fatal 'Gasping Syndrome' in premature infants."
- Aluminum. The label says the product "contains aluminum that may be toxic," with the risk mainly in patients with impaired kidney function on prolonged parenteral therapy.
Neither is a purchasing decision. But your clinicians should know what's in the product you stock, and these warnings are why.
How should it be stored?
- 20°C to 25°C (68°F to 77°F), with excursions allowed to 15°C–30°C.
- "PROTECT THE PRODUCT FROM LIGHT," in the label's own capitals (American Regent label). Keep vials in the carton, not loose on a counter.
Where should a clinic buy it?
From a licensed wholesaler that gives you DSCSA transaction information and statements. B12 is a prescription injectable like any other, and the same supply-chain rules apply. Here's how to verify a wholesaler in four checks.
Ordering checklist
- Order by full NDC. The 1 mL, 10 mL, and 30 mL versions all have different NDCs. If you're not sure what one is, look it up.
- Pick vial size from 28-day use, not monthly use.
- Write the discard date on every opened multiple-dose vial.
- Keep it in the carton until it's used.
- Ask what expiration date ships before you buy a box of 25.
