Progesterone injection USP, what everyone calls "progesterone in oil," is 50 mg/mL of progesterone in sesame oil, with benzyl alcohol as a preservative. It comes in a 10 mL multiple-dose vial and is labeled for intramuscular use only (Eugia label). Sensitivity to sesame oil or seeds is a labeled contraindication.

It's a simple product to stock. The two things that trip clinics up are the oil and the 28-day clock.

What's in the vial?

From Eugia's labeling (label; package and storage):

  • Progesterone 50 mg per mL (500 mg per 10 mL)
  • Sesame oil as the vehicle
  • Benzyl alcohol 10% as a preservative
  • A clear, colorless to yellowish, oily solution
  • 10 mL multiple-dose vial, boxed individually, NDC 55150-306-10
  • Vial stopper not made with natural rubber latex
  • For intramuscular use only

What's it labeled for, and who shouldn't get it?

The labeled indications are amenorrhea and abnormal uterine bleeding due to hormonal imbalance, in the absence of organic pathology such as submucous fibroids or uterine cancer (Eugia label). Fertility practices also stock it. How it's used is a prescriber decision.

The labeled contraindications include (Eugia label):

  • Current or past thrombophlebitis, thromboembolic disorders, or cerebral apoplexy
  • Liver dysfunction or disease
  • Known or suspected breast or genital malignancy
  • Undiagnosed vaginal bleeding
  • Missed abortion
  • Known sensitivity to progesterone injection or to sesame oil or seeds

That last one is why the vehicle matters. If a patient can't have sesame, this product isn't a simple swap. That's a conversation for the prescriber and the pharmacist.

The 28-day problem

It's a multiple-dose vial, so once it's opened, CDC's rule applies: date it and discard it within 28 days unless the manufacturer states another date (CDC). Write the discard date on it, not the date you opened it (The Joint Commission).

For a clinic that gives a few injections a week from a 10 mL vial, the 28-day clock can run out before the vial does. Count your real use per opened vial before you set reorder quantities. More in our single-dose vs. multiple-dose vials guide.

How should it be stored?

Eugia's label says store at 20°C to 25°C (68°F to 77°F), USP controlled room temperature (label). As with any parenteral, inspect for particles and discoloration before use. It's an oily solution, so look closely.

Ordering checklist

  • Order by NDC. Different manufacturers' products can differ in vehicle and preservative. If you're not sure what an NDC is, look it up.
  • Flag the sesame oil for your clinicians and patient intake.
  • Match vial count to 28-day use, not monthly volume.
  • Store at room temperature, not in the fridge with your other fertility meds.
  • Ask what expiration date ships.
