Famotidine injection is 10 mg/mL, and it's a refrigerated product: store at 2°C to 8°C, protected from light, in the carton until use. In Mylan's line, the 2 mL vial (20 mg) is single-dose with no preservative. The multiple-dose vials contain benzyl alcohol and have their own NDCs (Mylan label).
The fridge part is where clinics get caught. Famotidine shows up in the same procedure-area order as a dozen room-temperature drugs, so it's easy to put away in the wrong place.
What does it come in?
From Mylan Institutional's labeling (label):
Same drug and same concentration in all three. The difference is single-dose vs. multiple-dose, and the preservative.
How should it be stored?
Per the label (Mylan label):
- Store at 2°C to 8°C (36°F to 46°F). Protect from light, and keep it in the carton until use.
- If the solution freezes, bring it to room temperature and allow enough time for all components to dissolve.
What that means for purchasing:
- Receive it into the fridge the same day. Flag it on the packing list so it doesn't get shelved with the room-temperature drugs.
- Ask how it ships. Refrigerated items need cold-chain packaging.
What about after it's diluted?
The label says diluted famotidine is stable for 7 days at room temperature in several common solutions, including 0.9% sodium chloride, 5% or 10% dextrose, and lactated Ringer's. But because there are no sterility data after dilution, it says to refrigerate and use within 48 hours if not used immediately (Mylan label). Chemical stability and sterility aren't the same thing. The 48-hour limit is the one that governs. For more on those IV fluids, see our LR vs. normal saline guide.
Single-dose or multiple-dose?
If most of your use is one 20 mg vial per patient, the 2 mL single-dose vial is the clean choice, with no preservative and no 28-day clock. Multiple-dose vials fall under CDC's 28-day rule once opened unless the manufacturer says otherwise, and they belong to one patient if they go into the treatment area (CDC). More in single-dose vs. multiple-dose vials.
Ordering checklist
- Order by NDC. Single-dose and multiple-dose versions have different NDCs and different preservatives. If you're not sure what an NDC is, look it up.
- Refrigerate on receipt and log it with your fridge items.
- Keep it in the carton for light protection.
- Confirm cold-chain shipping before the first order.
- Ask before a substitute ships. A different manufacturer may change vial size or storage.
