Ondansetron injection, the generic for Zofran injection, is 2 mg/mL. The workhorse clinic package is a 2 mL single-dose vial (4 mg), sold in cartons of 25, labeled for intravenous or intramuscular use. In Apotex's labeling, the vials store anywhere from 2°C to 30°C and need to be protected from light (FDA label via FDALabel).
It's one of the easiest injectables to stock. Here are the few things worth checking anyway.
What does it come in?
From the Apotex labeling (FDA label):
Other manufacturers also sell multiple-dose vials, with their own NDCs and their own ingredient lists. If you're comparing, check whether a product is single-dose or multiple-dose before you compare prices. Our single-dose vs. multiple-dose vials guide explains why it matters.
How should it be stored?
The label says: "Store vials between 2° and 30°C (36° and 86°F). Protect from light" (FDA label).
That's a wide range, which makes life easy. But "protect from light" still means keeping vials in the carton, not loose in a drawer under the procedure-room lights.
What warnings are on the label?
Purchasing doesn't administer it, but you should know what clinicians are weighing when they ask for it. The label's warnings section covers (FDA label):
- Hypersensitivity reactions, including in patients who have reacted to other selective 5-HT3 antagonists
- QT prolongation
- Serotonin syndrome
- Myocardial ischemia
- Masking of progressive ileus and gastric distension
Single-dose means single-dose
A 2 mL single-dose vial is one patient, one injection. CDC says never to use single-dose vials for more than one patient and not to save leftovers, "even on the same patient" (CDC). If your clinicians regularly use part of a vial, that's a product-selection conversation, not a reason to stretch vials.
Ordering checklist
- Order by NDC. Single-dose and multiple-dose versions have different NDCs and different ingredient lists. If you're not sure what an NDC is, look it up.
- Keep cartons closed until use for light protection.
- Match carton count to use. A carton of 25 is the standard unit. Check the expiration date that ships if you're a low-volume user.
- Ask before a substitute ships. A different manufacturer may mean a different package or storage range.
